You will require a USI prior to obtaining the Statement of Attainment. 

You can create your USI at http://usi.gov.au/create-your-USI/Pages/default.aspx it only takes 5 minutes. Please bring your USI with you when you attend your training course.

Course Details:
The 1 day Forklift Course is relevant for any person wishing to obtain the Nationally Recognised High Risk Work Licence to operate a forklift (LF class).  Before you enrol ensure you review the detailed course instructions, Click Here
No practical driver training is provided as part of this course.  It is "assessment only". Participants sitting this course are required to provide written evidence of at least 40 hours of forklift driving experience in a warehouse-type environment over the last 4 weeks.  Your evidence must be handed in at check in.  Failure to do so will incur additional rescheduling fees of 50%.  
Participants need to complete a practical assessment including manoeuvring a forklift through an obstacle course (forwards and in reverse), lifting pallets into pallet racking and lifting various loads into pallet racking at height, without receiving any training.
Note: Due to changes in WorkSafe's assessment process, there is a MANDATORY pre-reading component (of approximately 2 hours) which is required to be completed before sitting this course.  This can be done via a PDF workbook. Students who have not completed the necessary pre-course study will need to reschedule to an alternate date, at a cost of 50% of the course fee.

What to Bring:
Strict ID requirements apply for this course.  You will need either: 1 x form of photo ID (Australian Driver's Licence, or any passport) plus 1 x form of non-photo ID (e.g. Medicare or ATM card); OR 1 x form of primary non-photo ID that includes your date of birth (e.g. birth certificate), plus 2 forms of secondary non-photo ID (e.g. Medicare, ATM or Health Care Card).  Must wear appropriate clothing, long trousers, enclosed shoes (preferably safety boots).
 
 
Assessment:
Participants must pass a written theory assessment (in English) and practical driving assessment in order to successfully complete this course.

Pre-requisites:
Participants need to be at least 18 years of age.  Must have a strong level of English language to be able to complete the written theory assessment and understand spoken instructions in the practical component.  Must have thorough current forklift driving experience and be able to provide written evidence of such.
   
Reasonable Adjustments:
Participants with learning difficulties may apply to conduct an oral theory assessment or may request extended time to complete their learning and assessment.  Please make this known at the time of booking.
